Thank you for registering your child/children for our 2017 Vacation Bible School. We know it will be a memorable experience as they learn to apply biblical principles to everyday situations. The theme for this year is "Bullying Activity a Roaring Standard" (B.A.R.S). While every effort will be made to safeguard your child/children safety; accidents and injuries may occur. By having your child/children participate in Vacation Bible School at the First Seventh Day Baptist Church of Toronto, you the parent or legal guardian agree to hold harmless, release, and indemnify the First Seventh Day Baptist Church of Toronto, its directors, employees, agents, associates, lessor, and subsidiaries from all liability relating to injuries that may occur during all activities held at 49 Brydon Drive, Toronto, ON, M9W 4N3 on August 1-4, 2017. By signing this agreement, you agree to hold the First Seventh Day Baptist Church of Toronto entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. You also acknowledge the risks involved in a Vacation Bible School Program. These include, but are not limited to, falls, cuts, bruises, choke hazards, emergency First Aid/CPR, etc. You further acknowledge that your child’s/children participation is voluntarily, and that all risks have been made clear. Additionally, you acknowledge that your child/children do not have any conditions that will increase his/her likelihood of experiencing injuries while engaging in prescribe activities. You also give implied consent for your child/children to receive medical treatment that may be deemed necessary in the event of injury, accident, and/or illness during this event. You agree that medical treatment may include, but is not limited to First Aid/CPR, transportation to hospital, and medical treatment by a duly licensed physician/nurse; and agree to hold harmless, release, and indemnify the First Seventh Day Baptist Church of Toronto, its directors, employees, agents, associates, lessor, and subsidiaries from all liability relating to such said medical treatment. You further consent, without limitations or reservations to photographs/pictures/videos of your child/children during the course of the First Seventh Day Baptist Church of Toronto Vacation Bible School Program; and to the use of such said photographs/pictures/videos for publication. By signing below, you forfeit all right to bring a suit against the First Seventh Day Baptist Church of Toronto for any reason. |
